Intraoperative Ultrasound and Contrast-Enhanced Ultrasound (CEUS) Features in a Case of Intradural Extramedullary Dorsal Schwannoma Mimicking an Intramedullary Lesion.

چکیده
Introduction ▼ Primary spinal tumors are relatively rare lesions, representing 4–8 % of all central nervous system tumors (Duong LM et al. Cancer 2012; 118: 4220–4227). Magnetic resonance imaging (MRI) represents the gold standard for the diagnosis and assessment of intradural tumors. Nevertheless, sometimes MRI may not accurately differentiate between intramedullary and extramedullary tumors. The role of intraoperative ultrasound (ioUS) on the other hand is not yet well-defined and standardized for this surgical field. We report a case of a dorsal schwannoma in which preoperative MRI showed findings compatible with an intramedullary lesion with an exophitic component. However, ioUS with the adjunct of contrast-enhanced ultrasound (CEUS) helped to reveal the lesion to be completely extramedullary. To the best of our knowledge, this is the first reported case of use of CEUS during spinal tumor surgery.

متن کاملIntraoperative ultrasound assistance in treatment of intradural spinal tumours.
OBJECTIVE Currently, the standard practice to treat intradural spinal tumours involves microsurgical resection of the lesions. It is essential to be able to locate the lesion precisely to reduce the risk of neurological morbidity. The purpose of this study was to evaluate intraoperative ultrasonography (IOUS) in visualizing intradural spinal tumours, and assess its potential to improve surgical...
متن کاملTitle Page - Contrast-Enhanced Ultrasound of Nodular Splenic Extramedullary Hematopoiesis in a Patient with Underlying Primary Myelofibrosis.
Introduction ▼ Extramedullary hematopoiesis (EMH) is a pathophysiological compensatory response due to insufficient erythrogenesis seen in leukemia and myeloproliferative diseases such as myelofibrosis (MF). It consists of diffuse proliferation of erythroid precursors outside the bone marrow in reticuloendothelial organs such as the spleen, liver and lymph nodes. Nodular EMH, a morphological va...
متن کاملIntraoperative contrast-enhanced ultrasound for brain tumor surgery.
BACKGROUND Contrast-enhanced ultrasound (CEUS) is a dynamic and continuous modality that offers a real-time, direct view of vascularization patterns and tissue resistance for many organs. Thanks to newer ultrasound contrast agents, CEUS has become a well-established, live-imaging technique in many contexts, but it has never been used extensively for brain imaging. The use of intraoperative CEUS...
متن کاملAncient Schwannoma of the Trigeminal Nerve Mimicking High Grade Lesion
Ancient schwannomas rarely found in the central nervous system and ancient change in an intracranial schwannoma (intracranial portion of the trigeminal nerve) is extremely rare. A seventy-year man presented with headache of 3 months duration and one episode of generalized tonic-clonic seizures. Imaging findings were suggestive of left temporal high grade lesion. During surgery there was well...
